Patients per day: 17-20

Payer mix: 72% private pay/commercial; 27% Medicare; 2% Medicaid

BLS - Required

Ability to start within 2-3 weeks- REQUIRED 

Must have a clean background for expedited credentialing

$500 referral bonus received for any referral who works 30 days.
